Youth Outreach Worker

Responsibilities

• Provide active outreach to at-risk youth (12-21) in the community
• Support clients in navigating health and social service systems and advocating for their needs
• Deliver group and individual support, including health promotion, harm reduction, and life skills education
• Assist clients with essential paperwork such as birth certificates and health card applications
• Accompany clients to medical appointments to provide support
• Maintain and develop partnerships with community organizations and service providers
• Compile, enter, and report confidential client data within agency systems
• Participate in program planning, team meetings, and case conferences

Requirements

• Completion of a 2-year post-secondary program in Health or Social Services
• At least one year of experience working in a community setting
• Skills in screening, intake, and assessment
• Knowledge of harm reduction, health education, and peer support models
• Understanding of issues affecting marginalized and vulnerable populations
• Experience working with newcomers and racialized communities
• Excellent interpersonal, presentation, and communication skills
• Ability to work independently and within a multi-disciplinary team
• Valid driver’s license and access to a vehicle for regular travel
• Proficiency in MS Office and related software
• Ability to work some evening or weekend shifts
