About the role
Responsibilities
- Deliver community-based services by meeting with young adults (ages 16-26) in their homes and community locations across Bucks County.
- Use the evidence-based Transition to Independence Process (TIP) model to support independence in education, employment, living situations, and personal wellbeing.
- Support young people in creating and following personalized "Futures Plans" to achieve their goals.
- Advocate for young adults in multi-system meetings and collaborate with families, schools, and community partners.
- Plan and facilitate group events that promote life skills, connection, and recreation.
- Complete clinical documentation and meet billable hour requirements in alignment with regulatory standards.
- Participate in case management reviews, weekly team meetings, and an on-call rotation.
Requirements
- Bachelor’s degree in social work, psychology, criminal justice, or a related human services field.
- Valid US driver’s license held for at least two years (excluding permit time).
- Access to a reliable vehicle with valid registration, inspection, and insurance for travel within Bucks County.
- Ability to travel and transport youth throughout the community.
- No serious traffic convictions within the past three years.
Preferred Qualifications
- Previous work or internship experience within mental health or community-based human services.
- Knowledge of various mental health diagnoses and developmental needs, including Autism Spectrum Disorder.
