About the role

Responsibilities

  • Develop, implement, and maintain the Wound, Ostomy, and Continence Program for home health care patients
  • Assess and treat patients with ostomies, incontinence, complex wounds, and pressure sores
  • Recommend appropriate products, devices, and supplies to achieve cost-effective, positive outcomes
  • Act as the primary resource for referral and care regarding wound and ostomy management
  • Provide staff education, including preparing reference materials and conducting in-service programs
  • Participate in Quality Improvement (QI) activities and the development of clinical policies and procedures
  • Document all patient care in accordance with agency policies and nursing standards

Requirements

  • Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) with a commitment to obtain BSN within three years
  • Current Illinois Registered Professional Nurse license
  • AHA Basic Life Support (BLS) certification
  • Completion of an approved course in Enterostomal or Wound, Ostomy, Continence nursing
  • Active driver’s license and auto insurance
  • Strong interpersonal, collaborative, and analytic skills

Preferred Qualifications

  • 2 years of experience as an Enterostomal/Wound, Ostomy, Continence Nurse
  • WOCN certification
  • Previous experience in home health care
