About the role
Responsibilities
- Diagnose and treat diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers
- Stage pressure injuries according to NPIAP guidelines
- Utilize advanced wound care products including collagens, alginates, foams, and silvers
- Manage wound vacs and other DME, such as pressure relieving/reducing devices
- Provide primary and preventative care for patients in long-term care and skilled nursing facilities
- Manage residents with acute changes in condition and provide advanced care planning
- Prescribe appropriate medications, including controlled substances, in accordance with State Law
- Collaborate with an interdisciplinary care team and primary care physicians to coordinate patient care
- Communicate regularly with nursing home staff regarding changes in condition or care plans
Requirements
- Master’s degree in Family Health (ANCC/AANP accredited)
- Active and unrestricted Nurse Practitioner license
- National board certification (ANCC or AANP)
- Valid driver’s license and willingness to travel locally within the service area
- EMR proficiency and strong documentation skills
- Basic Life Support (BLS) certification
- Ability to manage patients with advanced illness and multiple chronic conditions
Preferred Qualifications
- Wound care certification or willingness to obtain certification
- 2–5 years of experience as a Nurse Practitioner
- Experience working in a long-term care, home health, or public health setting
- Bilingual fluency
- Experience with immunizations and point-of-care testing
