About the role

Responsibilities

  • Coordinate and monitor Relay implementation at assigned hospital emergency departments
  • Provide technical assistance and training to ED staff on program policies
  • Use lived experience with substance use to support patients following non-fatal overdoses
  • Conduct overdose prevention trainings and naloxone distribution
  • Engage patients within 24 hours of hospital discharge
  • Assist patients with linkage to care and community resources for up to 90 days
  • Maintain detailed records of patient contacts and program data

Requirements

  • Baccalaureate degree and two years of community work experience, OR
  • High school diploma and six years of community work experience
  • At least one year of relevant community-centered experience
  • Ability to work effectively with culturally diverse populations
  • Willingness to travel within the five boroughs

Preferred Qualifications

  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) or Peer specialist (CCAR) training
  • Two or more years of experience in the human services field
  • Familiarity with harm reduction principles and medication for opioid use disorder
  • Strong communication and organizational skills
