About the role
Responsibilities
- Coordinate and monitor Relay implementation at assigned hospital emergency departments
- Provide technical assistance and training to ED staff on program policies
- Use lived experience with substance use to support patients following non-fatal overdoses
- Conduct overdose prevention trainings and naloxone distribution
- Engage patients within 24 hours of hospital discharge
- Assist patients with linkage to care and community resources for up to 90 days
- Maintain detailed records of patient contacts and program data
Requirements
- Baccalaureate degree and two years of community work experience, OR
- High school diploma and six years of community work experience
- At least one year of relevant community-centered experience
- Ability to work effectively with culturally diverse populations
- Willingness to travel within the five boroughs
Preferred Qualifications
- Credentialed Alcoholism and Substance Abuse Counselor (CASAC) or Peer specialist (CCAR) training
- Two or more years of experience in the human services field
- Familiarity with harm reduction principles and medication for opioid use disorder
- Strong communication and organizational skills
