Responsibilities
- Offer support and care in the home in accordance with individually adapted care plans
- Contribute to a safe and caring home environment for customers
- Document and report care interventions and observed changes in customer condition
- Participate in team meetings and communicate with colleagues to ensure integrated care
Requirements
