Responsibilities

• Offer support and care in the home in accordance with individually adapted care plans
• Contribute to a safe and caring home environment for customers
• Document and report care interventions and observed changes in customer conditions
• Participate in team meetings and communicate with colleagues to ensure integrated care

Requirements

• Empathetic and flexible personality
• Strong communication skills
• Ability to work independently and take initiative
• Proficiency in spoken and written Swedish
• Valid B-driver's license
• Valid work permit for Sweden
• Relevant experience in home care or similar care activities is a merit
