Triage RN Overnight

Responsibilities

• Triage after-hours calls using established protocols to promote positive outcomes and reduce preventable ER admissions
• Assess patient needs and perform in-person visits as necessary, including admissions and routine visits
• Communicate with physicians and hospice medical directors regarding status changes and to obtain orders
• Coordinate transitions to inpatient settings or hospice houses, providing complete SBAR and medication reports
• Use disease-management principles, OASIS knowledge, and clinical judgment to guide after-hours decision-making
• Review after-hours referrals, assign staff appropriately, and communicate updates to all disciplines
• Document all calls, communications, and clinical actions in real time in the patient record and triage log
• Provide exceptional customer service to patients, families, caregivers, and partners

Requirements

• Registered Nurse license in good standing with the New Hampshire Board of Nursing
• At least two years of home health or acute care experience
• Prior triage experience
• CPR/BLS certification
• Associate degree or higher in Nursing
• Ability to meet driving, background, and competency requirements

Preferred Qualifications

• Bachelor's degree or higher in Nursing
• Recent experience in homecare or hospice settings
• Prior experience with on-call responsibilities
