Transitional Care Coordinator

Responsibilities

• Coordinate with the interdisciplinary team in the provision of social services and discharge planning
• Complete comprehensive psychosocial assessments of patients
• Act as a liaison between patients, families, internal departments, and community referral partners
• Assist patients and families in accessing concrete services and resources
• Respond to crisis situations involving patients, families, and staff
• Facilitate individual counseling for patients and families as needed
• Participate in the development of patient care plans and treatment interventions
• Maintain accurate and timely documentation in compliance with federal and state regulations
• Serve as a patient advocate regarding rights, responsibilities, and insurance benefits

Requirements

• Bachelor’s degree in Social Work or a related human services field
• Minimum of one year of supervised social work experience in a healthcare setting
• Minimum of one year of experience with case management and discharge planning
• Understanding of Medicare and Medi-Cal guidelines
• Strong interpersonal and communication skills
• Effective time management and organizational skills

Preferred Qualifications

• Knowledge of medical terminology
