About the role

Responsibilities

  • Provide individualized professional support and case coordination to young people transitioning from Out-of-Home Care to independence
  • Develop young persons' natural networks and assist them in maintaining connections to culture, country, family, and community
  • Broker access to financial support and resources through the Home Stretch WA service offer
  • Deliver tailored support services using a culturally safe, strengths-based, and trauma-informed approach
  • Collaborate with stakeholders including Department of Communities staff, foster families, carers, and community members
  • Promote the wellbeing and self-determination of young people up to the age of 21

Requirements

  • Degree or Diploma in Social Welfare, Social Science, or a similar field
  • Minimum of two years of experience in a similar case management role
  • Proven experience in assessment, counseling, and crisis intervention for vulnerable youth
  • Strong knowledge of Aboriginal culture and ability to work in a culturally safe manner
  • Excellent interpersonal, networking, and communication skills
  • High-level computer literacy and ability to work flexibly in a solutions-focused manner
  • Full driver licence with no restrictions
  • Ability to undergo Child Protection Screening Checks and national police history checks
