Transfer Center RN

Responsibilities

• Serve as the primary point of contact for facilities and providers requesting patient transfers or access to services.
• Coordinate all admissions and transfers, managing bed placement and organizational patient flow.
• Communicate with referring facilities regarding transfer status and bed assignments.
• Monitor institutional capacity to assess the impact on patient flow and resource management.
• Implement strategic goals related to capacity management and service-line development.
• Maintain professional relationships with referring providers and facilities.

Requirements

• Bachelor’s Degree in Nursing or currently enrolled in a nursing program (must enroll within 6 months).
• Minimum of 5 years of Acute Care Nursing experience.
• Current and active New Hampshire Registered Nurse license.
• American Heart Association Healthcare Provider BLS certification.
• Strong organizational, multi-tasking, and prioritization skills.
• Ability to work a flexible schedule, including night, weekend, and holiday assignments.

Preferred Qualifications

• Previous supervisory or leadership experience.
• Proficiency in leading diverse teams within a complex management structure.
• Experience with Operational Excellence Guiding Principles (e.g., Strategy Deployment, Visual Management).
