Supportive Housing Case Manager

Responsibilities

• Conduct assessments and interviews with clients to determine eligibility for services
• Develop and update action plans, case notes, and essential client documentation
• Provide ongoing life skills development to assist clients in improving independence
• Deliver specialized interventions for clients at risk of eviction
• Connect clients to medical, mental health, and other supportive counseling services
• Guide clients through applications for SSI, SSDI, Medicaid, Medicare, and SNAP
• Establish a network of community services and conduct appropriate referrals
• Perform home visits and collaborate with property management for wellness checks
• Track and report outcomes and provide data updates as directed
• Travel throughout the area to provide transportation to clients as needed

Requirements

• Bachelor's degree in social services or related field with 1 year of experience in supportive housing or independent living services
• OR a High School Diploma/GED with 2 years of experience in supportive housing or independent living services
• Valid driver's license, insurance, and access to a vehicle
• Ability to lift up to 50 lbs
• Ability to perform CPR/First Aid

Preferred Qualifications

• Bilingual skills
• Proficiency with Microsoft Office, Zoom, and mobile devices
• Strong time management and proactive problem-solving skills
