About the role
Responsibilities
- Implement student Care Coordination Model and manage Individualized Care Plans
- Conduct student and family intake processes including demographic data collection
- Provide one-on-one mentorship and facilitate linkages to external resources
- Oversee intake documentation and manage Support Services protocols
- Manage work plans, scheduling, and training for Alumni Youth Care Coordinators
- Coordinate Genius and Family activity events and meetings
- Input and manage youth data within Salesforce MIS
- Train staff and volunteers on human service topics and best practices
Requirements
- Experience working with urban, underserved, or low-income youth
- Experience mentoring Black male youth
- High comfort level with technology
- Strong leadership and organizational abilities
- Ability to adapt to a dynamic, rapidly-changing environment
- Commitment to the life success of Black males and men of color
Preferred Qualifications
- Bachelor's Degree
- Master's in Social Work (MSW)
- 3+ years of experience in direct service case management
- 3+ years of experience in youth services
- Excellent group facilitation and presentation skills
