About the role
Responsibilities
- Assist individuals and families through education and support to develop and achieve personal visions within self-directed services.
- Manage service operations, coordination, and family collaboration to ensure services are tailored to individual needs.
- Facilitate person-centered planning to identify short and long-term goals and initiate actions to make them a reality.
- Provide information, referrals, and advocacy regarding local, state, and federal resources.
- Maintain accurate record-keeping and documentation on statewide and internal platforms.
- Facilitate communication between service providers, individuals, families, and the agency.
- Ensure compliance with state regulations, federal statutes, and agency policies.
Requirements
- Associate's Degree required.
- Two years of experience in human services, developmental services, special education, or a related field.
- Valid NH driver's license and an acceptable driving record.
- Access to a personal vehicle for frequent travel and home visits.
- Computer competency for documentation and data management.
- Ability to communicate effectively in both verbal and written formats.
