Support Service Coordinator of Self-Directed Services

Responsibilities

• Assist individuals and families through education and support to develop and achieve personal visions within self-directed services.
• Manage service operations, coordination, and family collaboration to ensure services are tailored to individual needs.
• Facilitate person-centered planning to identify short and long-term goals and initiate actions to make them a reality.
• Provide information, referrals, and advocacy regarding local, state, and federal resources.
• Maintain accurate record-keeping and documentation on statewide and internal platforms.
• Facilitate communication between service providers, individuals, families, and the agency.
• Ensure compliance with state regulations, federal statutes, and agency policies.

Requirements

• Associate's Degree required.
• Two years of experience in human services, developmental services, special education, or a related field.
• Valid NH driver's license and an acceptable driving record.
• Access to a personal vehicle for frequent travel and home visits.
• Computer competency for documentation and data management.
• Ability to communicate effectively in both verbal and written formats.
