Support Coordinator

Responsibilities

• Assist NDIS participants in coordinating and implementing support services based on their NDIS plan using a person-centred approach
• Build strong relationships with participants, families, NDIS coordinators, planners, and service providers
• Strengthen and enhance clients' ability to coordinate supports and participate in the community
• Regularly monitor NDIS plans to ensure services are delivered within allocated budgets
• Ensure mainstream services (housing, education, justice, health) meet their service obligations to clients
• Provide the client and the NDIA with reports on outcomes and success indicators

Requirements

• Unrestricted driver's licence and willingness to travel as required
• NDIS Workers Screening Check
• VIC Working with Vulnerable People Check
• National Criminal History Record Check
• Strong written and computer skills for creating plans, reports, and correspondence
• Excellent verbal communication and engagement skills
• Demonstrated experience managing competing priorities and delivering outcomes within timeframes
