Student Social Work Clinical Placement

Responsibilities

• Support the hemodialysis healthcare team in providing safe and effective dialysis therapy under the direct supervision of a licensed social worker
• Conduct psychosocial assessments to identify patient strengths and areas of need affecting rehabilitation and treatment outcomes
• Participate in collaborative care planning to identify interventions that improve patient quality of life and treatment adherence
• Provide psycho-educational counseling and supportive guidance to patients, including those seeking transplant
• Assist patients with advance directives, Do Not Resuscitate orders, and referrals to community resources such as home health or vocational rehabilitation
• Educate patients and families regarding kidney disease, treatment modalities, and psychosocial issues related to End Stage Renal Disease (ESRD)
• Collaborate with financial coordinators to help patients understand billing, financial responsibilities, and available assistance programs
• Maintain updated patient resource lists and assist with administrative support tasks related to social work services

Requirements

• High School diploma or G.E.D.
• Currently enrolled in an accredited Social Work education program
• Commitment to meeting CSWE (Council on Social Work Education) educational standards and field education hour requirements
