Staff RN - Home Health

Responsibilities

• Deliver nursing care to patients in a home health setting using the nursing process and established protocols
• Perform OASIS/comprehensive assessments and complete clinical documentation in a timely manner
• Coordinate and facilitate the patient plan of care by communicating with physicians, medical staff, and interdisciplinary teams
• Supervise and document the home health aide plan of care every two weeks
• Provide individualized patient and family education regarding care and safety
• Manage weekly case management schedules and oversee LVNs and CHHAs
• Participate in on-call rotations and respond to calls within required timeframes
• Adhere to infection prevention guidelines and organizational safety standards

Requirements

• Registered Nurse (RN) License
• BLS certification
• College Diploma in Nursing
• Minimum of 1 year of clinical healthcare experience
• Valid Driver's license, auto insurance, and reliable transportation
