Staff Registered Nurse

Responsibilities

• Deliver nursing care to patients within a home setting using the nursing process and established medical plans of treatment.
• Perform OASIS/comprehensive assessments and ensure timely, accurate documentation.
• Coordinate and facilitate the patient plan of care by communicating with physicians, medical staff, and interdisciplinary team members.
• Oversee and supervise the care provided by LVNs and CHHAs, including documenting home health aide plans every two weeks.
• Provide individualized patient and family education regarding health status and treatment protocols.
• Manage weekly case management schedules and participate in weekly case conferences and staff meetings.
• Adhere to infection prevention guidelines, safety standards, and regulatory compliance.
• Participate in on-call rotations, responding to calls within 20 minutes.

Requirements

• Registered Nurse (RN) License.
• BLS certification.
• College Diploma in Nursing.
• Minimum of 1 year of clinical healthcare experience.
• Valid Driver's license, auto insurance, and reliable transportation for home visits.
