Speech Therapist

Responsibilities

• Evaluate patients and perform appropriate therapy interventions
• Develop a comprehensive plan of care and coordinate multi-disciplinary care
• Perform clinical documentation and visit notes using Homecare Homebase
• Maintain continued communication with physicians, care team members, patients, and families
• Implement and maintain SLP protocols
• Manage patient treatment and outcomes directly

Requirements

• Current Texas Speech-Language Pathology License
• One year of SLP experience
• Current Texas driver's license
• CPR certification
• BLS Certification

Preferred Qualifications

• Experience with Homecare Homebase
• Previous Home Health experience
