About the role
Responsibilities
- Provide a full range of Speech Language Pathology Services as ordered by physician
- Participate in patient care planning with other home care personnel
- Conduct initial history, evaluations, and skilled procedures
- Consult with physicians regarding treatment changes and progress
- Complete initial evaluations, treatment plans, and goals for physician approval
- Manage recertifications every 60 days as required
- Write progress notes for each visit, reevaluations, and discharge summaries
Requirements
- Master's or Doctoral degree in Speech Language Pathology
- State licensure as a Speech Language Pathologist
- Knowledge of medical terminology
- Reliable transportation and valid automobile insurance
Preferred Qualifications
- Two years of experience as a Speech Therapist
- Home Health experience
