Speech Language Pathologist (SLP) - Home Care

Responsibilities

• Develop and implement appropriate plans of care (POC) in partnership with patients, representatives, and caregivers
• Perform thorough assessments to identify patient limitations, goals, and potential discharge environments
• Document accurate and ongoing assessments, treatments, interventions, and patient responses to care
• Engage patients and caregivers in meaningful education related to the care plan
• Coordinate care with ordering providers and other members of the interdisciplinary healthcare team
• Participate in agency projects, committees, and QAPI programs as requested
• Safely operate a personal vehicle to travel throughout the service area for patient visits and meetings

Requirements

• Master's degree in speech-language pathology, communications sciences and disorders, or communications disorders
• Graduate of an approved ASHA speech-language program with a Certificate of Clinical Competency (CCC-SLP)
• Current New Hampshire speech-language pathology license
• Two years of inpatient, skilled/acute, or community health speech-language experience
• Valid driver's license, reliable transportation, and an acceptable driving record
