Requirements
Master's degree in speech-language pathology, ASHA certificate (CCC-SLP), New Hampshire speech-language pathology license, Two years inpatient or community health experience, Valid driver's license and reliable transportation
About the role
Responsibilities
- Develop and implement appropriate plans of care (POC) in partnership with patients, representatives, and caregivers
- Perform thorough assessments to identify patient limitations, goals, and potential discharge environments
- Document accurate and ongoing assessments, treatments, interventions, and patient responses to care
- Engage patients and caregivers in meaningful education related to the care plan
- Coordinate care with ordering providers and other members of the interdisciplinary healthcare team
- Participate in agency projects, committees, and QAPI programs as requested
- Safely operate a personal vehicle to travel throughout the service area for patient visits and meetings
Requirements
- Master's degree in speech-language pathology, communications sciences and disorders, or communications disorders
- Graduate of an approved ASHA speech-language program with a Certificate of Clinical Competency (CCC-SLP)
- Current New Hampshire speech-language pathology license
- Two years of inpatient, skilled/acute, or community health speech-language experience
- Valid driver's license, reliable transportation, and an acceptable driving record
