Speech Language Pathologist (SLP) - Home Care

Responsibilities

• Conduct patient evaluations and develop individualized plans of care
• Implement and adjust evidence-based treatment plans for communication, swallowing, and cognition
• Educate patients and caregivers on therapeutic strategies to support independence
• Complete timely and accurate clinical documentation
• Communicate with physicians and collaborate with the interdisciplinary care team
• Participate in care conferences and agency quality initiatives
• Safely travel throughout the Granite VNA service area to provide home-based care

Requirements

• Master's degree in speech-language pathology, communications sciences and disorders, or communications disorders
• ASHA certificate of clinical competency (CCC-SLP)
• Current New Hampshire speech-language pathology license
• Two years of inpatient, skilled/acute, or community health speech-language experience
• Valid driver's license, reliable transportation, and an acceptable driving record
