About the Company

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model. Our mission is to dramatically improve patient access to home health care through clinical excellence and technology that reduces documentation burden for field clinicians.

Responsibilities

  • Complete comprehensive in-home SLP evaluations and deliver skilled SLP visits
  • Assess and treat aphasia, cognitive-communication impairment, and dysphagia
  • Build and execute a clear plan of care with measurable, functional goals
  • Drive multidisciplinary coordination by communicating changes in condition to the care team
  • Coach patients and caregivers with practical strategies and routines
  • Document promptly, thoroughly, and efficiently

Requirements

  • Active Michigan SLP license in good standing
  • Strong clinical reasoning and ability to operate independently in the field
  • Ability to identify red flags and escalate changes in condition
  • Strong caregiver education and communication skills
  • Current driver’s license and comfort with daily travel across assigned territory

Preferred Qualifications

  • Prior home health experience
  • High-signal neuro experience (stroke, aphasia, cognitive impairment)
  • Dysphagia-related experience appropriate for home health
