About the Company
Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model. Our mission is to dramatically improve patient access to home health care through clinical excellence and technology that reduces documentation burden for field clinicians.
Responsibilities
- Complete comprehensive in-home SLP evaluations and deliver skilled SLP visits
- Assess and treat aphasia, cognitive-communication impairment, and dysphagia
- Build and execute a clear plan of care with measurable, functional goals
- Drive multidisciplinary coordination by communicating changes in condition to the care team
- Coach patients and caregivers with practical strategies and routines
- Document promptly, thoroughly, and efficiently
Requirements
- Active Michigan SLP license in good standing
- Strong clinical reasoning and ability to operate independently in the field
- Ability to identify red flags and escalate changes in condition
- Strong caregiver education and communication skills
- Current driver’s license and comfort with daily travel across assigned territory
Preferred Qualifications
- Prior home health experience
- High-signal neuro experience (stroke, aphasia, cognitive impairment)
- Dysphagia-related experience appropriate for home health
