Speech and Language Therapist

Responsibilities

• Conduct initial and follow-up assessments for patients with dysphagia in homes and nursing homes
• Develop and implement individualized treatment plans
• Provide interventions for adult patients with swallowing difficulties and communication disorders
• Work closely with multidisciplinary teams to ensure effective care
• Liaise with family members and carers to support treatment and care plans
• Deliver follow-up assessments remotely when appropriate
• Maintain accurate and up-to-date patient records
• Participate in team meetings and supervision sessions

Requirements

• HCPC registered Speech and Language Therapist
• Extensive experience in dysphagia management in adult community settings
• Valid UK driving license and access to a car
• Enhanced DBS check or willingness to obtain
• Strong communication and interpersonal skills
• Ability to work independently and within a multidisciplinary team
