Responsibilities

• Provide specialist geriatric care within a multi-professional team
• Conduct patient assessments, treatments, and follow-ups for conditions such as heart failure, COPD, diabetes, dementia, osteoporosis, or reduced muscle strength
• Work within a patient-centered framework in an outpatient clinic setting

Requirements

• Specialist certification in Geriatrics
• Experience working with multi-morbid patients in geriatric departments
• Proficiency in the Swedish language (spoken and written)
• Strong communication skills and ability to work both independently and in a team

Preferred Qualifications

• Experience working in a multi-professional environment including nurses, occupational therapists, physiotherapists, dietitians, and counselors
