About the role
Responsibilities
- Collect, organize, and communicate medical data to develop time-sensitive treatment plans for Department of Defense beneficiaries
- Assess patients via telephone using established protocols to provide cost-effective care
- Establish patient care priorities and monitor progress toward health goals
- Oversee discharge-planning activities to ensure proper sequencing of patient care
- Identify patient needs for referrals to appropriate healthcare providers or facilities
- Negotiate and administer service agreements to ensure compliance with care goals
- Utilize VA/DOD Clinical Practice Guidelines to ensure evidence-based standardized care
- Manage information systems and technology to track and communicate practice outcomes
Requirements
- U.S. Citizenship
- Secret or Top Secret Clearance (or ability to obtain)
- Current, full, active, and unrestricted license as a Registered Nurse or Licensed Clinical Social Worker
- Minimum of two years of nursing or case management experience
- Case Manager certification from a recognized organization (or ability to obtain within one year)
- Basic Cardiac Life Support (BCLS) certification by the American Heart Association
- Fluency in English (oral and written)
