About the role

Responsibilities

  • Provide professional health nursing principles and clinical care management to Department of Defense (DOD) beneficiaries
  • Collect, organize, and communicate data relevant to primary health assessments and detailed medical histories
  • Develop time-sensitive treatment plans and oversee discharge-planning activities
  • Assess patients via telephone using established protocols to provide cost-effective care
  • Establish patient care priorities and monitor progress toward stated health goals
  • Negotiate and administer service agreements to ensure compliance with care goals
  • Utilize VA/DOD Clinical Practice Guidelines to ensure evidence-based standardized care
  • Manage information systems and technology to track and analyze population health outcomes

Requirements

  • U.S. Citizenship
  • Active, unrestricted Registered Nurse (RN) license in a U.S. state or territory
  • Minimum of two (2) years of nursing experience
  • Secret or Top Secret security clearance (or ability to obtain one)
  • Case Manager certification from a recognized organization (or ability to obtain within one year)
  • Current Basic Cardiac Life Support (BCLS) certification by the American Heart Association
  • Strong communication skills for interacting with military health system stakeholders
