Social Worker

Responsibilities

• Assess patients to evaluate service needs and formulate treatment plans
• Implement intervention plans with patients, families, and inter-professional teams to facilitate behavioral or environmental changes
• Reassess and modify intervention plans as required
• Liaise with community agencies and complete applications for Alternate Level of Care (ALC) facilities
• Provide clinical teaching to social work students, medical students, and professional colleagues
• Participate in relevant research activities and departmental projects
• Exchange information regarding patients' psycho-social functioning and healthcare history with the inter-professional team

Requirements

• Master's Degree in Social Work
• Registration with the Ontario College of Social Workers and Social Service Workers
• Experience in ambulatory care
• Experience working with chronic kidney disease populations
• Excellent verbal and written communication skills
• Strong interpersonal, time management, and organizational skills

Preferred Qualifications

• Membership in the Ontario Association of Social Workers
• Three years of experience working with an inter-professional team in a hospital setting
• Experience working with individuals with complex chronic health conditions or geriatric populations
• Research experience or experience in student education/supervision
• Knowledge of a second language
