Social Worker

Responsibilities

• Assess patients to evaluate service needs, identify problems, and formulate treatment plans
• Implement and modify intervention plans for patients, families, and inter-professional teams
• Liaise with community agencies and complete applications for Alternate Level of Care (ALC) facilities
• Provide clinical teaching to social work students, medical students, and professional colleagues
• Participate in relevant research activities and projects
• Exchange information regarding patients' psycho-social functioning and healthcare history

Requirements

• Master's Degree in Social Work
• Registration with the Ontario College of Social Workers and Social Service Workers
• Experience in an acute care setting
• Excellent verbal and written communication skills
• Strong interpersonal, time management, and organizational skills

Preferred Qualifications

• Membership in the Ontario Association of Social Workers
• Three years of experience working with an inter-professional team in a hospital setting
• Research experience or education
• Experience in student education or supervision
• Knowledge of a second language
