About the role
Responsibilities
- Support Spine/Neurosurgery inpatient units and other clinical areas by assessing patient needs and formulating treatment plans
- Implement intervention plans with patients, families, and interprofessional teams to facilitate behavioral or environmental changes
- Facilitate family and team meetings to coordinate care
- Liaise with community agencies and complete applications for Alternate Level of Care (ALC) facilities
- Provide clinical teaching to social work students and colleagues
- Participate in relevant research activities and projects
Requirements
- Master's Degree in Social Work (MSW) or equivalent
- Registration with the Ontario College of Social Workers and Social Service Workers
- Knowledge of the Health Care Consent Act and Advance Care Planning
- Knowledge of policies and legislation related to social welfare institutions and services
Preferred Qualifications
- Membership in the Ontario Association of Social Workers
- 2-3 years of experience working with an interprofessional team in a healthcare setting
- Experience in research or student supervision
- Proficiency in a second language
