Social Worker, Registry Off-Shift

Responsibilities

• Develop and lead or co-lead support and psychoeducational groups for designated patient and family populations.
• Formulate realistic discharge plans in collaboration with interdisciplinary healthcare teams and community organizations, including DCFS.
• Perform case management functions, including information, referral services, and linkages to hospital and community resources.
• Serve as a consultant and educator to multidisciplinary teams regarding the biopsychosocial components of illness and hospitalization.
• Conduct accurate psychosocial assessments examining current problems and relevant environmental factors.
• Participate in multidisciplinary patient care rounds and data collection processes.

Requirements

• Master’s degree in social work from a CSWE-accredited school.
• Current Illinois Social Work License (LSW required).
• Demonstrated knowledge of the principles of growth and development over the lifespan.

Preferred Qualifications

• Licensed Clinical Social Worker (LCSW) designation.
• At least one year of previous clinical social work experience in an acute care hospital setting.
