Social Worker-MSW

Responsibilities

• Coordinate complex discharge planning needs for patients and their families in conjunction with the interdisciplinary team.
• Perform comprehensive psychosocial assessments to identify emotional, social, and environmental issues impacting patient outcomes.
• Provide crisis intervention, supportive counseling, and advocacy to assist patients and families with adjustments related to illness or hospitalization.
• Develop and implement discharge plans, including arranging transportation and coordinating community resources.
• Facilitate patient rights, including education on medical treatment, advanced directives, and self-determination.
• Serve as a resource to hospital staff regarding social issues such as APS, CPS, domestic violence, and mental health processes.
• Collaborate with Care Coordinators and physicians to ensure efficient patient throughput and continuity of care.
• Document all assessments, interventions, and communications in accordance with hospital policy and professional standards.

Requirements

• Master of Social Work (MSW) degree is required.
• Proven skills in psychosocial assessment and intervention.
• Experience in crisis intervention and managing complex patient needs.
• Strong ability to perform discharge planning and coordinate post-acute services.
• Excellent written and verbal communication skills.
• Ability to work both independently and as part of an interdisciplinary healthcare team.

Preferred Qualifications

• Previous social work experience in a healthcare or hospital setting.
