Social Worker (m/f/d) for Social Services

Responsibilities

• Advise patients and relatives regarding home and inpatient follow-up care
• Initiate and advise on measures for follow-up medical treatments
• Provide counseling on social law issues according to SGB
• Support and counsel in conflict and stress situations related to illness
• Advise on financial assistance
• Manage structured discharge planning for patients across various clinical departments

Requirements

• Diploma or BA in Social Work
• Experience in hospital social services or patient-facing sectors
• High level of social competence and resilience
• Empathic demeanor
• Reliability, strong communication, and organizational skills
• Independent and goal-oriented way of working
• Proficiency with digital media is desirable
