Responsibilities
- Conduct strength-based assessments to determine adults’ needs, risks, and eligibility for support
- Formulate, implement, and review person-centred care and support plans
- Collaborate with health professionals, community services, and partner agencies to coordinate interventions
Requirements
- Social Work England registration
- Eligibility to work in the UK

- Post-qualified experience
- Degree level or equivalent in Social Work
