About the role
Responsibilities
- Complete strengths‑based assessments and Care Act evaluations
- Ensure timely, person‑centred support planning that promotes independence, wellbeing, and choice
- Manage a varied and complex caseload including safeguarding enquiries and mental capacity assessments
- Coordinate with multiple agencies to ensure safe and effective outcomes
- Work collaboratively with health partners, community services, and families
- Facilitate smooth transitions and review care packages
Requirements
- Social Work England registration
- Eligible to work in the UK
- Full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
