Responsibilities
- Complete strengths‑based Care Act assessments and develop person‑centred support plans
- Manage a varied caseload, including safeguarding, risk assessment, and complex decision‑making
- Work collaboratively with health partners, providers, and families to promote independence and ensure safe, effective support
