Social Worker-LCSW-Clinic

Responsibilities

• Conduct comprehensive biopsychosocial assessments to understand patient needs, goals, and preferences, including cultural and linguistic factors.
• Facilitate behavioral change and provide tailored social and emotional support for patients, including counseling regarding end-of-life issues.
• Provide care management for behavioral health conditions, including crisis intervention and short-term clinical counseling.
• Coordinate care transitions between healthcare practitioners and settings, including securing medical or community-based appointments.
• Collaborate with interdisciplinary teams to promote continuous process improvement and clinical excellence.
• Perform screenings to evaluate patient needs for behavioral health and support services.
• Document patient progress and interventions in accordance with established standards and ICD codes.

Requirements

• Master’s degree in Social Work.
• California Licensed Clinical Social Work (LCSW) license.
• Two years of clinical social work experience.
• Ability to complete 5150 Certification Training within ninety (90) days of hire.

Preferred Qualifications

• Experience in managed care, medical, or behavioral health settings.
• Experience in crisis intervention, oncology, or palliative care.
