Social Worker LCSW-C

Responsibilities

• Assist patients, significant others, physicians, and healthcare team staff in understanding personal, emotional, environmental, and social factors related to health status.
• Contribute to the development of a comprehensive, integrated Plan of Care for patients on a daily basis.
• Instruct healthcare team members on available community resources to assist patients.
• Function as a Bereavement Coordinator when needed, supervising the provision of bereavement services and establishing plans of care to address bereavement needs.

Requirements

• Master's Degree from a school of Social Work accredited by the Council of Social Work.
• Current CPR certification.
• Licensed Social Worker (LCSW-C) in the state of residence.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• 1+ years of social work experience in a healthcare setting.

Preferred Qualifications

• Bereavement Coordination experience.
• Experience with establishing a plan of care for bereavement needs.
