Social Worker

Responsibilities

• Provide psychosocial services and counseling to dialysis patients and families
• Assess psychosocial needs, support care planning, and help patients adjust to treatment
• Collaborate with the interdisciplinary care team to improve quality of life and treatment outcomes
• Educate patients and staff on the emotional and behavioral impact of chronic kidney disease
• Connect patients with community, vocational, insurance, and financial resources
• Document assessments, interventions, and patient progress in accordance with policy
• Support quality improvement, patient satisfaction, and continuity of care activities

Requirements

• Master’s degree in Social Work
• State-required social work license
• 2–5 years of related experience
• Strong counseling, assessment, and care coordination skills
• Ability to work effectively with patients, families, and healthcare teams

Preferred Qualifications

• Experience working with dialysis, ESRD, or chronic illness populations
• Knowledge of transplant referral processes and community support resources
• Familiarity with quality improvement and patient education practices
