Social Worker

Responsibilities

• Provide psychosocial services and supportive counseling to in-center and home dialysis patients
• Conduct patient assessments to identify psychosocial status, strengths, and areas of need
• Collaborate with the interdisciplinary healthcare team to develop care plans and improve patient quality of life
• Educate patients and families on kidney disease, treatment modalities, and psychosocial impacts of ESRD
• Facilitate referrals to community resources, including vocational rehabilitation and home health services
• Assist patients with the transplant referral process and waitlist management
• Support patient understanding of advance directives and DNR status in collaboration with medical staff
• Monitor and report on quality indicators such as treatment adherence and quality of life trends
• Coordinate with insurance professionals to address patient coverage and financial assistance needs

Requirements

• Master’s degree in Social Work (MSW)
• Valid state-required social work license
• Knowledge of Social Work Theory of Human Behavior
• 0–2 years of related experience (entry-level)
