Social Worker

Responsibilities

• Undertake comprehensive psychosocial assessments and complex discharge planning, primarily within the aged care sector
• Provide patient and family inclusive practice throughout all stages of care
• Engage in family work, outpatient work, and group work
• Supervise students and contribute to a cohesive clinical team
• Demonstrate flexibility and adaptability within a dynamic clinical environment

Requirements

• Bachelor or Master of Social Work
• Eligibility for membership with the AASW
• Minimum of three years of relevant social work experience in a hospital, rehabilitation, or healthcare setting
• Well-developed expertise in assessment, intervention, and discharge planning
• Knowledge of aged care systems and compensable systems (e.g., TAC, Workcover, NDIS, Centrelink, VCAT)

Preferred Qualifications

• Clinical experience or specific knowledge in the area of aged care
