Social Worker Case Manager

Responsibilities

• Collaborate with a multidisciplinary team to initiate and support discharge planning from admission
• Assess psychosocial needs and identify barriers to discharge
• Coordinate community resources and manage patient cases
• Maintain timely documentation in the electronic medical record
• Lead management of complex discharge needs requiring clinical judgment
• Facilitate safe transfers to skilled nursing facilities, inpatient rehabilitation, and long-term care
• Arrange post-acute services including home health and hospice

Requirements

• Bachelor’s of Social Work (BSW) from a CSWE accredited program
• BLS/CPR Certification within 90 days of hire

Preferred Qualifications

• Master’s of Social Work (MSW) from a CSWE accredited program
• At least one year of hospital case management experience
• Case Management Certification
• Rural healthcare experience
• Experience with Cerner EHR
