About the role
Responsibilities
- Conduct comprehensive needs and risk assessments for complex and high-risk cases
- Develop, implement, and monitor individualized care and intervention plans
- Provide clinical supervision and guidance to junior staff on case management and interventions
- Monitor and evaluate effectiveness of case management to beneficiaries
- Review and approve case assessments and outcome plans
- Prepare detailed case notes, reports, and social reports
- Lead case conferences and other case-related discussions
- Provide home visit volunteer training
Requirements
- Degree and/or Master in Social Work and/or Counselling
- Registered with the Singapore Association of Social Workers (SASW) or Singapore Association for Counselling (SAC)
- At least 5 years of relevant experience in a community environment
- Bilingual with ability to communicate in local dialects preferred
