Social Medicine Registered Nurse

Responsibilities

• Deliver primary and episodic care in non-traditional environments including mobile health vans and community hubs
• Provide specialized wound care, chronic disease management, and harm reduction support
• Use autonomous clinical judgment to assess, monitor, and respond to patient needs
• Support mobile outreach and navigation for clients facing barriers to traditional healthcare
• Advocate for health equity by identifying systemic barriers to care
• Collaborate with interdisciplinary teams including Nurse Practitioners, Social Workers, and Community Health Workers
• Maintain accurate clinical documentation in electronic medical record systems

Requirements

• Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
• 3+ years of clinical experience, preferably in community health, primary care, harm reduction, or mental health
• Demonstrated expertise in chronic wound care and lower extremity assessment
• Valid Ontario Driver’s License (G-class)
• Ability to work with structurally marginalized populations using trauma-informed care
• Strong clinical assessment and autonomous decision-making skills

Preferred Qualifications

• BScN degree
• Additional certification in mental health, addictions, or complex wound management
• Experience driving a cargo van
