About the role
Responsibilities
- Deliver primary and episodic care in non-traditional environments including mobile health vans and community hubs
- Provide specialized wound care, chronic disease management, and harm reduction support
- Use autonomous clinical judgment to assess, monitor, and respond to patient needs
- Support mobile outreach and navigation for clients facing barriers to traditional healthcare
- Advocate for health equity by identifying systemic barriers to care
- Collaborate with interdisciplinary teams including Nurse Practitioners, Social Workers, and Community Health Workers
- Maintain accurate clinical documentation in electronic medical record systems
Requirements
- Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
- 3+ years of clinical experience, preferably in community health, primary care, harm reduction, or mental health
- Demonstrated expertise in chronic wound care and lower extremity assessment
- Valid Ontario Driver’s License (G-class)
- Ability to work with structurally marginalized populations using trauma-informed care
- Strong clinical assessment and autonomous decision-making skills
Preferred Qualifications
- BScN degree
- Additional certification in mental health, addictions, or complex wound management
- Experience driving a cargo van
