Responsibilities
- Coordinate services by connecting clients with community resources such as housing, healthcare, employment, and mental health services.
- Engage clients to build trusting relationships and assist in developing personalized action plans for stability and independence.
- Manage crises using empathy and de-escalation skills to ensure the safety and well-being of all individuals.
- Maintain accurate documentation, including case files and client progress tracking, in compliance with program guidelines.
- Provide services in a respectful, dignified, and non-judgmental manner aligned with the organization's mission.
Requirements
- High school diploma or GED.
- Experience or ability to perform service coordination and client engagement.
- Strong crisis de-escalation and intervention skills.
- Proficiency in documentation and reporting.
- Compliance with the organization's vaccination policy (including flu vaccination).
