Service Transition Manager

Responsibilities

• Lead and support a team to deliver high-quality client onboarding and transition outcomes
• Engage closely with clients and their support networks to ensure a positive entry into services
• Oversee the coordination of service commencement, ensuring alignment with funding, compliance, and organisational requirements
• Build and maintain strong relationships with internal teams and external partners to enable seamless service delivery
• Contribute to strategies that strengthen client acquisition, onboarding, and retention practices
• Support business development initiatives by identifying opportunities for service growth
• Provide guidance, coaching, and development support to build team capability
• Ensure all activities align with quality, safeguarding, and risk management frameworks

Requirements

• Experience in client relationship management and stakeholder engagement
• Proven ability to lead and develop teams to achieve high-quality outcomes
• Strong analytical, problem-solving, and conflict resolution skills
• Experience contributing to or delivering business development or growth initiatives
• Ability to navigate complex environments and make evidence-based decisions

Preferred Qualifications

• Tertiary qualification in Business, Marketing, or a related discipline
• Experience in disability services or a related sector
• Experience working in a not-for-profit environment
