Service Transition Coordinator

Responsibilities

• Coordinate the transition of new clients into Scope services to ensure a smooth onboarding experience
• Act as the central point of contact for clients, families, and stakeholders during the onboarding process
• Gather and document client goals, preferences, and support requirements in compliance with funding standards
• Coordinate onboarding activities including service matching, site visits, and funding arrangements
• Collaborate with internal teams and external stakeholders to ensure seamless service entry
• Maintain accurate records and documentation in line with regulatory and organisational requirements
• Identify opportunities to improve onboarding processes and enhance the client experience
• Adhere to quality, safeguarding, and risk management frameworks

Requirements

• Experience working with multiple stakeholders to deliver coordinated outcomes
• Strong organisational and time management skills with the ability to manage competing priorities
• Effective communication skills across various formats and audiences
• High level of professionalism, integrity, and accountability

Preferred Qualifications

• Experience in disability services, community services, or a related sector
• Experience working within a not-for-profit environment
