About the role
Responsibilities
- Manage clinical intake and admission coordination for Hillcrest Reactivation Centre and associated transitional care programs
- Coordinate client flow and maintain up-to-date registries, including daily site census and bed reports
- Communicate effectively with referral sources, external stakeholders, and families to ensure excellent customer service
- Participate in GIM/ALC rounds at acute care sites both in person and virtually
- Lead internal monthly data validation processes and generate reports on performance indicators
- Coordinate quality improvement activities, data collection, and program evaluations
- Support the development of educational and marketing initiatives for referral partners
Requirements
- Bachelor's degree in a health disciplines field
- Membership in good standing with a respective regulatory college
- Minimum of 2 years of experience in an inpatient environment
- Knowledge of healthcare legislation and standards of practice
- Advanced Excel skills and proficiency in Microsoft Word
- Proven experience in project planning, implementation, and evaluation
- Strong interpersonal, leadership, and verbal/written communication skills
Preferred Qualifications
- 3-4 years of experience specifically as an intake or flow coordinator
- Previous experience working in a community healthcare setting
