Service Coordinator

Responsibilities

• Manage clinical intake and admission coordination for Hillcrest Reactivation Centre and associated transitional care programs
• Coordinate client flow and maintain up-to-date registries, including daily site census and bed reports
• Communicate effectively with referral sources, external stakeholders, and families to ensure excellent customer service
• Participate in GIM/ALC rounds at acute care sites both in person and virtually
• Lead internal monthly data validation processes and generate reports on performance indicators
• Coordinate quality improvement activities, data collection, and program evaluations
• Support the development of educational and marketing initiatives for referral partners

Requirements

• Bachelor's degree in a health disciplines field
• Membership in good standing with a respective regulatory college
• Minimum of 2 years of experience in an inpatient environment
• Knowledge of healthcare legislation and standards of practice
• Advanced Excel skills and proficiency in Microsoft Word
• Proven experience in project planning, implementation, and evaluation
• Strong interpersonal, leadership, and verbal/written communication skills

Preferred Qualifications

• 3-4 years of experience specifically as an intake or flow coordinator
• Previous experience working in a community healthcare setting
