Senior Occupational Therapist

Responsibilities

• Deliver community-based occupational therapy interventions within the reablement pathway
• Complete functional assessments to support safe discharge and prevent hospital admissions
• Promote independence through reablement-focused practice
• Review progress and recommend appropriate equipment, adaptations, and onward support
• Work collaboratively with reablement staff, social care, and partner agencies
• Ensure timely, person-centred interventions aligned with Pathway 1 objectives

Requirements

• Degree or Diploma in Occupational Therapy
• HCPC registration
• Significant experience in community OT and reablement
• Ability to work autonomously at a senior level
• Strong assessment, review, and care-planning skills
