Senior Care Coordination Specialist

Responsibilities

• Conduct biopsychosocial assessments to evaluate mental health status, acuity levels, and functional status.
• Develop and implement comprehensive, multi-disciplinary individualized service plans for high-risk patients.
• Provide clinical interventions including individual and group therapy, crisis intervention, and patient counseling.
• Coordinate care by collaborating with medical providers, family members, and community support services.
• Perform medical case management tasks including client monitoring, advocacy, and referral to resources.
• Manage administrative requirements such as financial eligibility screening and maintaining documentation standards for grant compliance.

Requirements

• Master's Degree in Social Work, Counseling, or a related field.
• Valid New Jersey State License (LSW or LAC).
• Minimum of two (2) years of post-Master's professional experience.
• Proven experience working with patients in the areas of mental health and/or substance use.
• At least one (1) year of experience in managed care and/or case management.
• Proficiency with Microsoft Office and strong organizational and communication skills.

Preferred Qualifications

• At least two (2) years of clinical experience caring for individuals living with HIV/AIDS.
• Bilingual proficiency in Spanish.
• Working knowledge of ICD-10 and DSM-V coding.
