About the role
Responsibilities
- Conduct biopsychosocial assessments to evaluate mental health status, acuity levels, and functional status.
- Develop and implement comprehensive, multi-disciplinary individualized service plans for high-risk patients.
- Provide clinical interventions including individual and group therapy, crisis intervention, and patient counseling.
- Coordinate care by collaborating with medical providers, family members, and community support services.
- Perform medical case management tasks including client monitoring, advocacy, and referral to resources.
- Manage administrative requirements such as financial eligibility screening and maintaining documentation standards for grant compliance.
Requirements
- Master's Degree in Social Work, Counseling, or a related field.
- Valid New Jersey State License (LSW or LAC).
- Minimum of two (2) years of post-Master's professional experience.
- Proven experience working with patients in the areas of mental health and/or substance use.
- At least one (1) year of experience in managed care and/or case management.
- Proficiency with Microsoft Office and strong organizational and communication skills.
Preferred Qualifications
- At least two (2) years of clinical experience caring for individuals living with HIV/AIDS.
- Bilingual proficiency in Spanish.
- Working knowledge of ICD-10 and DSM-V coding.
